Central Louisiana Area Health Education Center (“CLAHEC,” “we,” “us”) is a nonprofit organization that helps students explore healthcare careers and supports healthcare professionals and communities across our 17-parish service area.

This policy explains what information we collect through clahec.org and our online program applications, how we use it, and the choices you have. By using our website, you agree to the practices described here.

Information you give us. We collect what you choose to send us, including when you:

– Apply for a student or clinical program (name, contact details, school, grade, parish, parent or guardian contact, and answers to application questions)

– Fill out our contact form or email or call us

– Register for an event, workshop or continuing education offering

– Apply for a job with CLAHEC

Information collected automatically. When you visit clahec.org, our website and service providers may record basic technical details such as your IP address, browser type, device type, pages visited, and the date and time of your visit. This helps us keep the site secure and understand which pages are useful.

We do not knowingly collect Social Security numbers, financial account numbers, or medical records through our website.

We use the information we collect to:

– Review applications and select, place and communicate with program participants

– Contact parents or guardians about a student’s participation, schedule and transportation

– Answer questions and respond to requests sent through our website

– Plan events and send program updates and reminders

– Report program results to our funders and partners, usually as combined numbers that do not identify individuals

– Keep our website secure and improve how it works

– Meet legal, grant and recordkeeping requirements

We do not sell, rent or trade your personal information.

Our online program applications are hosted by Jotform, a third-party form service. When you submit an application, your information is received and stored by Jotform on our behalf and then delivered to CLAHEC. Jotform’s handling of that information is also covered by Jotform’s privacy policy (https://www.jotform.com/privacy/).

Our website may also use other outside services, such as web hosting, event calendars, embedded maps or videos, and fonts. These providers may collect technical details like your IP address when their content loads. Links to other websites, including our partners’ sites and social media pages, are provided for convenience. We are not responsible for the privacy practices of those sites.

Many of our programs serve junior high and high school students. We collect a student’s information only to run the programs they apply for, and we ask for a parent or guardian’s contact information and consent as part of the application for any participant under 18.

We do not knowingly collect personal information online from children under 13 without a parent or guardian’s consent. If you believe a child under 13 has sent us personal information without that consent, please contact us and we will delete it.

We do not use student information for advertising, and we do not share it with anyone except as described in Section 7.

Cookies are small files a website saves in your browser. Our website uses cookies to make pages work properly and remember your settings. \[We also use Google Analytics to see, in general terms, how visitors use our site. Google Analytics collects information such as pages visited and time spent on the site; it does not tell us who you are. You can learn more or opt out at tools.google.com/dlpage/gaoptout

You can set your browser to refuse or delete cookies. Some parts of the site may not work as expected if you do.

Our site does not currently respond to browser “Do Not Track” signals, because there is no common standard for them.

We share personal information only when needed to run our programs or when required, including with:

-Program sites and partners— hospitals, clinics, universities and schools that host a program you are accepted into, limited to what they need to host you

-Funders and oversight agencies— the statewide Louisiana AHEC program office and state or federal grant agencies, as required for program reporting

-Service providers— companies that host our website, forms and email, who may use the information only to provide services to us

-Legal requirements— when required by law, court order or to protect the safety of participants, staff or the public

We use reasonable administrative, technical and physical safeguards to protect the information we collect. Our website uses encryption (SSL) to protect information in transit. No website or storage system is completely secure, so we cannot guarantee absolute security.

We keep personal information only as long as needed for the purpose it was collected, or as required by our funders and applicable law. When it is no longer needed, we delete it or store it in a form that no longer identifies you.

You may ask us to review, correct or delete personal information you have given us, or to stop sending you emails. You can unsubscribe from our emails at any time using the link in the message. Some records may need to be kept to meet grant or legal requirements; if so, we will tell you.

We may update this policy from time to time. The effective date at the top shows when it was last changed. Changes take effect when posted on this page.
