These Terms of Use apply to clahec.org and the online forms and applications linked from it (together, the “Site”), operated by Central Louisiana Area Health Education Center (“CLAHEC,” “we,” “us”). By using the Site, you agree to these terms and to our Privacy Policy. If you do not agree, please do not use the Site.

If you are under 18, please review these terms with a parent or guardian. A parent or guardian must complete or approve program applications for participants under 18.

You may use the Site to learn about CLAHEC, apply for programs, register for events and contact us. When using the Site, you agree not to:

– Submit false or misleading information, or apply on someone else’s behalf without their permission (a parent or guardian applying for their child is permitted)

– Try to access areas of the Site, accounts or systems you are not authorized to use

– Upload viruses or harmful code, or interfere with how the Site works

– Use automated tools to scrape or collect information from the Site

– Use the Site for any unlawful purpose or to harass or harm others

We may suspend or block access to the Site for anyone who violates these terms.

Submitting an application does not guarantee acceptance into a program. Programs, dates, locations and eligibility may change, and we may cancel or reschedule a program when needed.

Each program may have its own requirements, such as attendance, transportation, conduct rules, parent or guardian consent, or forms required by host sites. Those program-specific requirements apply in addition to these terms. If they conflict, the program’s own requirements control for that program.

Applications are submitted through a third-party form service. By submitting one, you confirm that the information you provide is true and complete to the best of your knowledge.

The text, graphics, logos, photos, videos and other material on the Site belong to CLAHEC or are used with permission. This includes the CLAHEC name and logo and our characters Pearl and Pierre. You may view, print and share Site content for personal, educational or noncommercial use, as long as you credit CLAHEC and do not change it. Any other use requires our written permission.

Partner and funder logos shown on the Site belong to those organizations and are shown to recognize our partnerships.

When you send us comments, stories, testimonials, photos or other material, you give CLAHEC permission to use, edit and share it for our programs and outreach, including on the Site and social media. We will ask for separate permission before publishing a participant’s name, photo or story, and we will ask a parent or guardian if the participant is under 18.

The Site links to other websites, including partner organizations, social media and our application form provider. We do not control those sites and are not responsible for their content, availability or privacy practices. Using them is at your own risk and subject to their terms.

Health and career information on the Site is for general education. It is not medical advice and does not replace care from a qualified healthcare provider. Do not delay or skip medical care because of something you read on the Site. In an emergency, call 911.

CLAHEC is not a medical provider through this Site, and using the Site does not create a patient relationship with CLAHEC or any partner organization.

The Site is provided “as is” and “as available.” We work to keep information accurate and current, but we do not promise that the Site will be error-free, uninterrupted or free of harmful code, or that information on it is complete or up to date.

To the fullest extent allowed by law, CLAHEC and its board, staff, volunteers and partners are not liable for any indirect, incidental or consequential damages arising from your use of, or inability to use, the Site. Some laws do not allow these limits, so they may not apply to you.

You agree to hold CLAHEC harmless from claims arising from your misuse of the Site or your violation of these terms.

These terms are governed by the laws of the State of Louisiana. Any dispute related to the Site will be handled in the state or federal courts serving Rapides Parish, Louisiana.

We may update these terms from time to time. The effective date at the top shows when they were last changed. Continuing to use the Site after changes are posted means you accept the updated terms.
